The address number of the participant as defined in the Participant file table (F08901).

A user defined code (08/QE) that indicates the initial event that qualified a participant for COBRA continuation coverage.  

The date of the first (initial) event that qualified a participant for COBRA continuation coverage.

The user defined code (table 08/QE) and description of the second event that qualifies the participant for COBRA continuation.  There 
cannot be a second qualifying event unless an initial qualifying event has previously been entered for the participant.

The date of the second qualifying event.

Qualifying Event Type O-original event, S-subsequent events.  This code is assigned by the system as qualified events are created.

A code that indicates the activity you want to perform.  Valid codes are:    A Add    C Change    I Inquire    D Delete    .     End the program    Blank 
Clear the screen  NOTE: Depending on how your company has set up action code security, you may not be authorized to use all action 
codes.

The date the election notice was mailed to the qualified beneficiary for the first qualifying event. The system automatically updates this date 
when you print the election form.

Code to define the type of COBRA Notice to create. COBRA Letter Type is defined by UDC 08U/CN.

COBRA Notice Reason Code.  Notice code used to define the reason for the COBRA notice. 

An option that specifies the type of processing for an event.

An alternate address number in the Address Book system. You can use this field for any secondary business address that relates to the 
primary address. For example:     o Ship-to address     o Law firm     o CPA     o Securities agent     o Bonding agent  If you leave this field blank on 
an entry screen, the system supplies the primary address from the Address Number field.
